PLEASE NOTE: NO ENTRY will be accepted without entry fee. ALL ENTRIES must be on this form. NO REFUNDS will be allowed to players withdrawing before or during the Championships.
The ENTRY FORM of any competitor or Association which is incomplete or gives false information to the questions asked, may render that competitor or Association liable to disqualification at any time before, during or after the event.
ALL EVENTS are open to players entered in a team event at this year’s Country Championships and to players who were nominated for a composite team on their association or club’s team entry form.
